I, the undersigned parent or legal guardian, give permission for my teen to attend this overnight event hosted by the Crossroads Teen Ministry. I understand that all reasonable precautions will be taken to ensure the safety of participants.
I agree not to hold the church, its staff, volunteers, or event partners liable for injuries or incidents that may occur during this event.
I understand my teen must follow all rules and they are expected to respect all leaders, peers and property. If my teen fails to do so, I understand they may be sent home if necessary.
In the event of an emergency, I authorize the event leaders to obtain medical treatment for my teen if I cannot be reached.
