I, the undersigned parent or legal guardian, give permission for my teen to participate in this Crossroads Teen Ministry event. I understand that all reasonable precautions will be taken to ensure the safety of participants; however, I acknowledge that participation may involve certain risks.
I agree not to hold the church, its staff, volunteers, or event partners liable for injuries or incidents that may occur during this event.
In the event of an emergency, I authorize the event leaders to obtain medical treatment for my teen if I cannot be reached.
